Right click on the file.
Open It's properties.
Choose the "Screen" Tab
Click on the "Fullscreen" option, and the dot should move from window to here.
Click Apply and Ok.

It should open correctly from now on.

Kainminter's suggestion was exactly right... but if it didn't work, here are some things to try:

make sure MIDI2BAM.EXE is not read-only (in the file properties)

make sure the folder that MIDI2BAM is installed in is not read only

look for a file called MIDI2BAM.PIF and delete it. Go into the properties of MIDI2BAM.EXE and choose full-scren again. This should re-create a new MIDI2BAM.PIF

It's a matter of fullscreening. The program can't run in a window. It has to be full-screened, and Alt+Enter does just that. When you get that error message, click "OK" and then, if the window's still open (which it should be), THEN hold Alt and press Enter.

Hmm....Then I'm lost. I don't know what else to tell you. I've never seen a case of that happening.
