Is there a way to "roll a dice" and have the possibility the hero hurts themself?

Such as accidentally poisoning themselves or slicing themself?

Sure. Look in the attack editor under "Chaining..."

The type of chaining you probably want is the "Instead Chain" which can replace the normal attack.

It might be interesting to add a bitset to "Chain only if attack fails/misses".
