From what they've said, I understand now. It's not a OHR thing, or an OS thing; it's literally your monitor saying it doesn't support a resolution that was set.
My guess is that the OS doesn't remember the precise settings of "OHR" at some times, and tries to set the video card back to the tweaked ModeX, and the hardware basically says it can't handle the resolution and/or refresh rate. The solution here is to not even attempt to switch video modes once you've gotten inside OHR.

Set your refresh rate down. Your monitor can't handle the current one you have set too well. _________________

Okay, I tried setting my refresh rate to Optimal.
I think it worked, because that error message don't appear anymore.
